I’m excited to work with you! This agreement helps us set the stage for a safe, relaxing, and effective massage experience.
This agreement outlines the expectations, boundaries, and purpose of the massage therapy services provided by MellieNMT My goal is to create a safe, professional, and healing environment where you can fully enjoy the benefits of therapeutic massage and bodywork.
1. Purpose of Agreement
This agreement helps us set clear expectations so every session is comfortable, safe, and focused on your well-being.
2. Scope of Services
Each session is tailored to your needs and may include a combination of techniques such as:
- Swedish Massage (Relaxation)
- Deep Tissue Massage
- Trigger Point Therapy
- Shiatsu Techniques
- Therapeutic Stretching
- Myofascial Release
- Neuromuscular Therapy
Session Details:
- Assessment: Each session includes 10–15 minutes for assessment and reassessment to ensure your session is personalized and effective. (Except for first time clients who receive extra time to allow for proper intake and assessment.)
- Undressing Guidelines: You may undress to your comfort level. Proper draping is maintained at all times to protect your privacy and ensure professionalism.
3. Session Duration Pricing
- First Time Session: 90 minutes : $120
- Follow-Ups:
- 60 min: $120
- 90 min: $160
- 120 min: $200
- Mobil Wellness Upgrade:
- $20 Within 10 miles
- $20 + ($3.50 per extra mile exceeding 10 miles)
Collaboration with Your Medical Care Team:
Massage therapy works best when combined with your overall health plan. I encourage involving your healthcare team when needed. Massage complements medical care, helping to reduce pain, improve mobility, and support overall wellness.
4. Professional Boundaries and Safety
- Therapeutic, Non-Sexual Environment: Massage therapy is strictly therapeutic. Any inappropriate behavior, including sexual comments or suggestive conduct, will not be tolerated.
- Zero-Tolerance Policy: Inappropriate behavior may result in termination of the session and documentation to ensure a safe environment for everyone.
5. Client Expectations and Consent
- Health Disclosure & Communication: Please provide all relevant health information honestly on your intake form and communicate any discomfort or changes during the session.
- Acknowledgement of Purpose: Massage therapy is intended to promote relaxation, reduce tension, and support healing. It is not a substitute for medical diagnosis or treatment, and you may need to consult your healthcare provider as necessary.
6. Consent and Agreement
By signing below, you acknowledge that you have read and understood this agreement. You consent to receive therapeutic massage services and agree to maintain a respectful and safe environment for yourself and the practitioner.
Your privacy is always respected here. The information you share is confidential and only used to personalize your sessions so we can reach your goals together. With my medical background, I follow HIPAA and other privacy policies to keep your health details safe.

I run my practice with respect for my time, your time, and the time of others. Please read carefully before booking:
Payments
- A 50% deposit of the service fee is required to book your appointment.
- The remaining balance is due on the day of service.
Cancellations & Rescheduling
- Cancel at least 24 hours before your appointment to receive your deposit back.
- Cancel within 24 hours of your appointment and your deposit is non-refundable.
- Emergencies happen—communicate with me right away if something comes up. No communication means no reimbursement.
No-Shows
You are allowed 3 no-shows maximum. After the third, I will terminate our therapeutic-client agreement.
