At Mellie NMT, I believe quality bodywork should be accessible whenever possible. While I do not bill insurance directly, I can provide a detailed superbill for eligible services that clients may submit to their insurance provider for possible out-of-network reimbursement.

Coverage and reimbursement vary by individual insurance plan.


What Is a Superbill?

A superbill is an itemized receipt that includes the information your insurance company may require to process a reimbursement request for out-of-network healthcare services.

Depending on your benefits, you may be eligible for partial reimbursement after submitting your superbill.


What Your Superbill Includes

  • Provider name and credentials
  • National Provider Identifier (NPI)
  • CAMTC Certification Number
  • Date and length of service
  • Service fees paid
  • Applicable CPT code(s)
  • Clinical documentation, when appropriate

Who May Benefit?

A superbill may be helpful if you:

  • Have out-of-network massage therapy benefits
  • Use a Health Savings Account (HSA) or Flexible Spending Account (FSA), when permitted by your plan
  • Have been referred by a licensed healthcare provider
  • Are seeking reimbursement for medically necessary massage therapy

Insurance plans differ significantly. Before scheduling, I encourage you to contact your insurance provider and ask:

  • Do I have out-of-network benefits for massage therapy?
  • Is a referral or prescription required?
  • Which CPT codes are eligible for reimbursement?
  • Are there any documentation requirements?
